I think you're being too hard on yourself. No body does it perfect, and although timing, first impressions, etc, are important, they aren't everything.
Yup. I've forgiven terrible first many-things because I liked someone.

If she'd been into you, the time spent together previously would have taken the edge off the fumble. It could have even made the first attempt cute.

This. It's probably the worst part for me. I have always had a very hard time connecting with others. I often find out what someone thinks of me is exactly opposite of what I thought. There wasn't an awkward moment in the first two dates that I could detect. I was even clueless about the missed kiss until she sent that text, which arrived while I was blissfully driving home thinking it was really going somewhere. The very real possibility I was misreading things so badly really makes me think I'll never overcome this problem I've been fighting for so many years. Don't get me wrong, I'm never going to quit.

Yes...but she also seems to be a poor communicator. There is nothing that says she couldn't go for a kiss if she wanted one (and her getting snippy about the "owe" comment was weird, remember). Chalk it up to the perfect storm of incompatibility.
